2026-2027 Season
2026-2027 Divisional Application

2026-2027 Enrollment Verification Form
To be completed by all teams for each semester of play. Due dates: Oct 1, 2026 and Feb 1, 2027. Teams must complete a new form for Nationals.
2026-2027 Graduate Student Form
Additional Enrollment Form, to be completed by all Graduate students, for each semester of play. Graduate students must also be listed and verified on each semester's Enrollment Verification Form.
2026-2027 School Acknowledgment Letter Form
To be completed by all teams, once per year.
